In order to register to the I.B.AHC portal, you must fill this form.

After you have registered, you can login to the documentation area of the portal at any time and download the Informed Consent Forms necessary for the enrolment to the I.B.AHC Biobank and Clinical Registry (both for the treating physicians and their patients), the forms for the requests to use the Biobank and the Clinical Registry for research and further documentation about I.B.AHC.

Warning! The registration to the I.B.AHC portal does not grant the access the I.B.AHC Biobank and Clinical Registry, for which you must first enrol, if you are a treating physician or a patient, or send a written using request to A.I.S.EA, if you are a researcher.
